Adoption Application
Please fill out this adoption application in its entirety.  All of the information is required.  Please look over the application
and make sure you have everything you need to fill it out properly.  Due to volunteer time constraints, incomplete
applications will NOT be processed or returned to you for completion.  Please double check you application BEFORE you
submit it to us.  Applicants will be called by a DMA Puppy Rescue volunteer as soon as possible after submission for an
application review.  If your application review is favorable, you will then be contacted by a DMA Volunteer who will arrange
for your home visit and phone interview.  Pending a favorable home visit, the application approval process will then be
completed.  
DMA Puppy Rescue adoptions are NEVER determined on a first come basis.  DMA Puppy Rescue aways chooses the
most compatible environment for each of our dogs.  Wait times may vary and depend on many factors.  Adopters must be
over 21 years of age.
We like many other organizations abide by the Humane Association's recommendation.  They  recommend the minimum
age of the youngest child, for ANY DOG adoption to have a relatively good chance of success, to be at least 8 years.  
There are basically two major reasons.
First, until that age a child cannot reliably understand proper behavior around a dog.  If you have a puppy that is very
submissive, it is likely to get hurt.  If you have a puppy that is less submissive, you stand the chance of a child getting
nipped, and the dog being labeled a "biter"  often costing the animal it's life.
The second reason, both child and puppy are developmentally growing.  Toddlers and young children tend to move
unpredictably and run around, and puppies are inclined to chase them. Children often respond by shrieking and other
high-pitched sounds.  This leads the puppy to chase even more, and often nip as if playing with a litter mate.
The child is not recognized by a puppy the same way that an adult  is, again it is developmental.  This can result into a
child who is afraid of the dog, and is truly not enjoying the experience.  The puppies/dogs are then either crated or
relegated to the yard, or taken to a shelter.
Having a dog should be enjoyable for a child.  But, it should be an adult dog, and one that understands children.

Some Vets will not disclose information about your pets until given permission by you.  
To speed up the process it is a good idea to call or fax your Veterinarian and give them
permission to release information about your past or current pets to us.
Dad's My Angel Puppy Rescue collects information for the purpose of assessing
adopter suitability.  We will not share, sell or otherwise distribute the personal
information of our applicants to any other organizations.  We will only disclose an
applicant's personal information to a third party if required by law.
